League Fee Per Team: $230.00

 

New Club Bond: $100.00 for each team. Held for 1 year.

Registrars Package: One of every form / map listed above and 1 roster for every team.

 PASSES: COACH and PLAYERS:

·         ALL coaches and assistant coaches, including U9 & U10 must have a pass with a picture.

·         All players MUST have a player pass and sign their own player pass (U11 – U19).

·        All players pictures must be up to date (not a photo copy and must be a front facing view).

·        All players pictures must be glued in the upper left corner of the pass

·         All clubs must make new player passes when your players enter    U11 – U13 – U15. This means up to date pictures.

·         All passes must be typed. Labels may be used.

·         We don’t require you to fill out the blue membership pass.

·         Label format: Line 1:  State:  MA

                                          Line 2:  Name:  Last – First – Middle Initial

                                          Line 3:  Date of birth:  Month – Day – Year

                                          Line 4:  Club Name___________Age Group:_U_____

                                          Line 5:   Team Name:_________Season:(Spring or Fall)  Year:

 
ROSTERS

1.     [    ] All rosters must be the same. Use only the roster supplied by the PVJSL office or Web site.                          2.  [    ] Return 3 copies of your roster on white paper, not stapled in the envelope supplied by the League

3.     [    ] All players that are dual rostered team must place an (X) to the left

              of his or her name. Rule 2d. Example:  (x) Tyler O’Connell.

CLUBS:

·        All paper work for your teams must be delivered at the same time

·        The paper work has to be delivered by an Executive Council member.
